We require an adoption application, interview and a home check before
placing a Dalmatian. Adoption fees vary by age.
Our Adoption Application and Foster Screening Form are listed below in Microsoft Word format and Adobe PDF format.
The PDF file cannot be filled out on line, and must be printed out
and faxed or mailed. The Word document can be filled out on your
computer and emailed back to us, or can be printed and sent by fax or US
Mail.
